Who is asking

Two things, and neither of them is about your idea. We need a name for the agreement and an address to reach you at. Nothing about what you are building is asked here, and nothing about it is asked until the agreement is in place.

You will notice there is nowhere here to describe your venture. That is deliberate. The one thing the agreement exists to protect is the one thing we will not take before it is signed.

Whoever will be named as the counterparty on the agreement.
Only if the agreement should name an entity rather than you.
Optional, and deliberately a list rather than a box: it tells us the shape of the work without telling us the idea.
